Requirements
For a selected patient scenario (I work at acute care facility), identify related NANDA, NIC, and NOC elements. Describe in detail the data, information, knowledge, and wisdom that guided you. The scenario is one that you choose and is in a context familiar to you so that you can provide the detail requested and apply your learning from this point forward.
PREPARING THE PAPER
1. Required texts may be used as references, but a minimum of three sources must be from outside of course readings.
2. All aspects of the paper must be in APA format as expressed in the 6th edition.
3. The paper (excluding the title page and reference page) is 3–5 pages in length.
4. Ideas and information from professional sources must be cited correctly.
5. Grammar, spelling, punctuation, and citations are consistent with formal academic writing.
Some general pointers:
• Paper should be no more than 3-5 pages in length
• Do not include more than 1-2 direct quotes
• Use at least 3 sources/references

So, start with an introduction – what are you going to discuss.
Then describe your patient scenario. Give me enough details to help me see what is going on. For example, I used to work in the emergency department. I might choose a patient whom I triaged who came in with a severe sore throat, high fever, and unable to swallow. I would then give the details about his presentation.
Next, I would describe the DIKW framework AND how I used it to help figure out what this patient needed. I would describe the data and information in the situation, then how I applied knowledge to know how to triage this patient (I thought he might have epiglottis). Finally, because of wisdom, I was aware of the danger that doing a throat culture represented for this patient. Because of wisdom, when I saw the medical student do a throat culture, I made sure that the nurse assigned to this patient was aware of this. She then moved the crash cart to the patient’s room, and we were ready when the patient needed an emergency tracheostomy because his throat had swollen shut.
Then, discuss how you might use NANDA, NIC, and NOC to develop a care plan for this patient. So, for mine, it might be Risk for Impaired Gas Exchange related to throat occlusion manifested by stridor, and cyanosis, as the main nursing diagnosis. I would then go on to choose (and tell why for each) the appropriate interventions according to NIC and the appropriate outcomes according to NOC. Be sure to use the actual NIC and NOC terms as they are presented by NIC and NOC. Do not use ones that you come up with yourself. This paper is focused on standardized terminology so be sure to actually use the standardized terms.
Finally, write a conclusion. Be sure to include insights you gained from doing this paper.
Note: for much of this paper, it can be in first person. Just please try to avoid words such as “I believe” or “I thought”. You are writing this paper so I know it is what you believe or think.
